Caregiver Self-Assessment

Take the self-assessment to determine the level of burden and level of benefit you receive from providing care. A Family Consultant can review the self-assessment with you to identify ways to alleviate burden. Includes planning ahead, arranging care, managing stress, dealing with family issues, learning new care tasks, and about your role as caregiver.

Discharge Planning Resources for Caregivers

The Next Step in Care website is part of the United Hospital Fund campaign to build strong partnerships between family caregivers and health care professionals, especially during transitions between care settings.  The “What Do I Need as a Family Caregiver?” guide helps families develop an after-discharge care plan.

Your Discharge Planning Checklist” is available on the Medicare.gov Web site. Print versions should be available for order in June. It will also be translated into Spanish in the near future.  The checklist was tested with consumers, family caregivers, clinicians (including discharge planners), doctors.  We also received feedback from the QIOs, hospital industry representatives, and patient advocacy groups.
