Geriatric Care Management Services

Our care management service is fee-based -- the amount depends on your service choices.

Finding and evaluating services for older adults is something you’ve probably never done before.  Yet the choices you make can have an immense impact on your quality of life or that of someone dear to you. Through our Geriatric Care Management program, licensed social workers offer comprehensive, personalized advice on senior options in the Twin Cities metro area, including housekeeping and chore services, meals, transportation, adult day care, home care, and housing with services.

Office Consultations
Our geriatric care manager can help you plan for your own future or discuss potential housing and service options. Family members often seek office consultations when they want to discuss the needs of an older family member who resides elsewhere or when their older relative isn’t yet willing to be assessed.

Assessment and Recommendations
Our geriatric care manager visits the older adult at home to evaluate existing abilities and needs through an interview, often with family present. The assessment is multidimensional and covers health status, functional status, cognitive functioning, emotional status, environment, legal and financial situation, and support system.  A written summary identifies the strengths and needs of the older adult and family and offers options for addressing needs. The care manager then reviews the recommendations with the older adult and family.

Service Planning and Coordination
After doing an assessment and making recommendations, the geriatric care manager works with the older adult and/or family to create a plan for addressing needs. The plan may include home care such as housekeeping and personal care, community services such as transportation and adult day services, as well as involving family and friends in daily tasks. Once the plan is in place, the care manager can make specific arrangements to get the services started. If a move is indicated, the care manager can tour housing options with a client and set up the move.

Ongoing Monitoring and Coordination
Through visits and phone contacts, the geriatric care manager monitors care and services, provides emotional support, stays informed, and responds as needed. Communications and periodic updates with family members are arranged. As client needs change, plans are revised and services adjusted. Care managers can help in additional ways--by facilitating family meetings, attending doctor appointments, completing paperwork, and arranging for home maintenance.
