How We Help
Caring for an older adult can be rewarding, but understanding the aging process can also be complex and confusing. You and your family member may be facing new and challenging situations and you may even find it difficult to handle the normal tasks of daily living. You both may feel isolated, overwhelmed, worried or even depressed.
We provide formal assessments, education, resources, and professional services to help you and your family member cope, gain insight, reduce burden, and restore balance to your life.
We offer two types of services to help you evaluate your current situation and identify your next steps:
Caregiver Services
These services are focused on the caregiver, the person who is taking care of an aging family or friend.
When you help an elder, you still have your own life to manage. It is normal to feel overwhelmed, frustrated, and even isolated at times. Our Family Consultants will help support you.
Care Management Services
These services are focused on the elder.
Our professional Care Managers help you develop a plan of care by providing an assessment of physical, emotional and cognitive functioning. In addition, the Care Manager can help address environmental matters and living arrangements, identify and coordinate support services, and monitor ongoing needs.

Caregiver Self-Assessment
Take the self-assessment to determine the level of burden and level of benefit you receive from providing care. A Family Consultant can review the self-assessment with you to identify ways to alleviate burden. Call 651-234-2262 to talk to a trained Specialist who can match you with a Family Consultant in your area.
Professionals
Whether you are working with someone who is a caregiver to an elder, or directly with the elder, you understand the challenges they are facing. You are already acting in their best interests but you may see them struggling and in need of additional help.
