It is the policy of Eldercare Partners to provide services to all persons that meet service eligibility criteria without regard to race, color, national origin, handicap, sexual orientation, or age.

Caregiver Services

Initial Complimentary Consultation

•An Introductory phone call from the Family Consultant to get acquainted and set a time for a face to face meeting.

Caregiver Coaching 3 Month Package

•Up to two in-person visit(s) at your convenience with the professional Family Consultant.
•A formal caregiver assessment which will be reviewed by you and the Family Consultant.  The assessment will explore your strengths, needs, and goals.  It will be used to create a customized caregiver plan that includes goal setting, problem solving, coaching and ongoing support to reach established goals.
•Follow-up calls from the Family Consultant to check on your status.
•Unlimited access to the Family Consultant via email and phone consultation.
•A reassessment visit (either in person or over the phone) to document progress and agreed upon future needs.
•A final meeting (either in person or over the phone) is offered to provide closure to the coaching experience and equip you with the necessary tools to empower you on your caregiving journey.
•A survey is completed to receive your feedback.
•Travel time and independent research time are included in the package rate, and not billed separately.

Hourly Caregiver Coaching

•After a 3 month package of coaching has been completed, ongoing assistance from the Family Consultant is available to pursue goals and address changing needs.
•Services are available in-person or via phone and email support.
•Travel time and independent research time will be billed at the hourly rate.

Family Meeting Facilitation

•Services are available to help promote communication and decrease misunderstandings within the family structure.
•Helps families understand their current situation and plan for the future with realistic expectations.
•Family Meeting Facilitation is done in-person, and average time is 2 – 4 hours.
•Travel time and independent research time will be billed at the hourly rate.

Consultation

•Consultation services are available without a purchased package or formal assessment.
•Services may include advisory on a specific situations or topics such as:  training or education on managing risk factors, dealing with changing identity and expectations within the caregiving role, family dynamics, disease management, managing difficult behaviors, communicating with health care providers, navigating the healthcare or long-term care system, building a support system, or addressing legal and financial issues.
•Services are available in-person or via phone and email support.
•Travel time and independent research time will be billed at the hourly rate.

Services for Seniors

Geriatric Care Management - Written Care Plan Package

•Up to two in-person visit(s) at the family’s convenience with the professional Family Consultant.
•A formal assessment of the senior which will explore physical, emotional and cognitive functioning, activities of daily living, legal/financial issues, and informal/formal support.
•Follow-up calls or emails from the Family Consultant to clarify issues or gather additional information.
•Based on the results of the assessment, the Family Consultant will develop a customized geriatric care plan and recommendations.  The care plan will address physical, emotional, and cognitive health, environmental matters, legal/financial matters and identify support services for both the senior and the family caregiver.
•A final meeting (either in person or over the phone) is offered to review and explain the care plan and provide closure to the experience.
•A survey is completed to receive your feedback.
•Travel time and independent research time are included in the package rate and not billed separately.

   
Ongoing Hourly Geriatric Care Management

•After a written care plan package has been completed, ongoing assistance from the Family Consultant is available to adjust the care plan as needs change, or coordinate and monitor ongoing services, etc.
•Services are available in-person or via phone and email support.
•Travel time and independent research time will be billed at the hourly rate.
