Care Management:

Trying to manage care for an elder can be very confusing. You may know that a situation needs to improve, but it can be hard to identify how or even why. The following are sample reports, given to Care Management clients after an assessment has been completed by a trained Care Manager.  The reports help identify issues as well as offer useful solutions and resources, giving the client and their family a place to start, and goals to focus on.
