Who We Are

Eldercare Partners is a nonprofit organization providing award-winning Caregiver Services as well as Geriatric Care Management to seniors and their family caregivers. 

We partner with our member organizations. 

Eldercare Partners unites three of the most respected nonprofit organizations in the Minneapolis-St. Paul metropolitan area of Minnesota. The Eldercare Partners member organizations have deep community roots, having been in service from 30 to over 100 years.  In 1999 they came together to create Eldercare Partners with a mission to improve the lives of older adults and their family caregivers.  United with our member organizations, Eldercare Partners offers greater capacity, expertise, and experience than any one organization could offer alone.

We partner with other professionals. 

By working with community-based organizations, we expand our services to non-English speaking and underserved populations. We participate in grant projects, community coalitions and research studies to raise awareness, enhance quality, and understand caregiver needs. We provide trainings that teach skills and techniques to other professionals, and we have worked with U.S. Senator Amy Klobuchar and U.S. Senator Al Franken as they bring family caregiver issues to the national level.  By partnering with other professionals, our voice is stronger, our message is united, and our services are enhanced.
